Could that be do to something connected in the wrong way? Also, there was a bug with Object Emitter in an old version of 2012 that made RF crash. It was fixed, so you might need a patch for your version if still sticking to 2012. In case you are just using the SPH and don't plan on looking onto grap...

Ar you using an updated plugin?

Also, remember the object might be coming into max in a wrong scale. Try scaling it up and down. Last but not least, check if the Axis setup in Realflow reflects the one in Max. Finally, try exporting the mesh as Alembic.

Those things are very likely to happen in RF2013 if an object has reversed normals. Check all your objects inside Realflow if the normals are pointing into the correct direction.

The reason for that might have a similar reason as with Houdini: Apple implemented its OpenGL 4.1 driver using the core profile. Houdini requires the compatibility profile, which everyone else provides (Nvidia, AMD, Intel). As a result, Houdini still uses OSX's OpenGL 2.1 driver, which hasn't change...

Actually, the Hybrido2 mesher works based on the RPC (particle) files. You could try doing so, but keep some important things in mind. What kind of water are you after?

As a basis, you can use the MR Dielectric material or a Mental Ray Standard material with a glass preset. Change the IOR to 1.33 and you are close to water. However, this is just a simple base, you will most certainly have to put some extra effort into that.

Have you tried to remove all the translations from the pitcher geometry and keep just the rotation for simulating. You could then create a mesh and parent that mesh to the pitcher geo inside your 3d app. Doing so, the liquid inside the pitcher would move correctly, again. Although this might not wor...

You might limit the effect of your script to an animated cube, but you will have to script the method that checks if particles are inside that cube by yourself.
